INTRODUCTION: There is a limited understanding of the early nutrition and pregnancy determinants of short-term and long-term maternal and child health in ethnically diverse and socioeconomically vulnerable populations within low-income and middle-income countries. This investigation programme aims to: (1) describe maternal weight trajectories throughout the life course; (2) describe child weight, height and body mass index (BMI) trajectories; (3) create and validate models to predict childhood obesity at 5 years of age; (4) estimate the effects of prepregnancy BMI, gestational weight gain (GWG) and maternal weight trajectories on adverse maternal and neonatal outcomes and child growth trajectories; (5) estimate the effects of prepregnancy BMI, GWG, maternal weight and interpregnancy BMI changes on maternal and child outcomes in the subsequent pregnancy; and (6) estimate the effects of maternal food consumption and infant feeding practices on child nutritional status and growth trajectories. METHODS AND ANALYSIS: Linked data from four different Brazilian databases will be used: the 100 Million Brazilian Cohort, the Live Births Information System, the Mortality Information System and the Food and Nutrition Surveillance System. To analyse trajectories, latent-growth, superimposition by translation and rotation and broken stick models will be used. To create prediction models for childhood obesity, machine learning techniques will be applied. For the association between the selected exposure and outcomes variables, generalised linear models will be considered. Directed acyclic graphs will be constructed to identify potential confounders for each analysis investigating potential causal relationships. ETHICS AND DISSEMINATION: This protocol was approved by the Research Ethics Committees of the authors' institutions. The linkage will be carried out in a secure environment. After the linkage, the data will be de-identified, and pre-authorised researchers will access the data set via a virtual private network connection. Results will be reported in open-access journals and disseminated to policymakers and the broader public.

INTRODUCTION: Cardiovascular disease is the main cause of general and premature death of adults aged 30-69 years in Brazil and around the world. Unhealthy food environments have been implicated as one of the factors associated with cardiovascular disease morbimortality because they affect people's health conditions and nutrition. This study aims to explore the association between unhealthy food environments (deserts/swamps) and premature cardiovascular disease mortality in the Brazilian population. METHODS: This is an ecologic study using data from 5,558 Brazilian municipalities in 2016. The cardiovascular disease mortality data were obtained from the Mortality Information System of the Ministry of Health. The study on mapping food deserts in Brazil, developed by the Interministerial Chamber of Food and Nutrition Security, was used to evaluate the physical dimension of food access. The authors calculated the standardized rates of premature general and specific cardiovascular disease (stroke and ischemic heart disease) causes of death in the same period. To characterize food environments, the density of unprocessed and ultraprocessed foods per 10,000 population in tertiles was used. Crude and adjusted negative binomial regression models were used to study the associations of interest. RESULTS: After the necessary adjustments (human development index, gross domestic product per capita, unemployment rate, Gini index and Family Health Strategy coverage), it was found that municipalities with low unprocessed food supply were at the highest risk of increased mortality among women with ischemic heart disease (rate ratio first tertile: 1.08 [95% CI=1.01, 1.15]). Conversely, the municipalities where there was a greater offer of ultraprocessed foods showed a higher risk of death from cardiovascular diseases (rate ratio second tertile: 1.17 [95% CI=1.12, 1.22]; rate ratio third tertile: 1.20 [95% CI=1.14, 1.26]), from strokes (rate ratio second tertile: 1.19 [95% CI=1.13, 1.25]; rate ratio third tertile: 1.22 [95% CI=1.15, 1.30]), and ischemic heart disease (rate ratio second tertile: 1.19 [95% CI=1.12, 1.25]; rate ratio third tertile: 1.22 [95% CI=1.13, 1.29]). CONCLUSIONS: This study's findings show an increase in the risk of cardiovascular disease, stroke, and ischemic heart disease mortality, especially in the municipalities where there was a greater offer of ultraprocessed foods. Initiatives aiming to minimize the effects of these food environments are urgently needed in the Brazilian context.

RESUMO. Objetivo: Analisar a associação entre insatisfação com a imagem corporal e autoestima de adolescentes matriculados na rede pública estadual de ensino fundamental em Salvador-Bahia. Método: Participaram deste estudo de corte transversal 860 adolescentes de 10 a 19 anos de idade, de ambos os sexos, matriculados do 7º sétimo ao 9º nono ano em 12 escolas de médio porte da rede pública estadual do ensino fundamental em turno integral, do município de Salvador, Bahia. A autoestima foi avaliada por meio da Escala de Autoestima de Rosenberg e a insatisfação com a imagem corporal pelo Body Shape Questionnaire. Dados demográficos e socioeconômicos, maturação sexual, estado antropométrico, nível de atividade física e sedentarismo foram também avaliados. A associação entre insatisfação com a imagem corporal e autoestima foi analisada pela regressão multinominal para variáveis politômicas utilizando o programa estatístico SPSS, versão 23.0. Resultados: Entre os adolescentes, 24,8% tinham moderada autoestima e 27,3% apresentaram os menores escores desse constructo. A insatisfação com a imagem corporal foi identificada em 25,2% dos adolescentes. A análise de associação revelou que adolescentes com insatisfação com a imagem corporal possuem 5,61 (IC 95% 3,08-10,24) vezes maior chance em manifestar baixa autoestima para o sexo feminino e 3,83 (IC 95% 2,03-7,25) vezes para o sexo masculino. Ainda entre as meninas, a insatisfação com a imagem corporal permaneceu associada positivamente à moderada autoestima após ajuste do modelo (OR: 2,31; IC 95% 1,21-4,41). Conclusão: Foi observada alta prevalência de níveis baixos e moderados de autoestima e que a insatisfação com a imagem corporal influenciou a baixa autoestima em ambos os sexos e a moderada entre as meninas.

Métodos: Participó en esta sección transversal un total de 860 adolescentes de 10 a 19 años, de ambos sexos, de 7º a 9º grado, matriculados en doce escuelas medianas en instituiciones públicas estatales de enseñanza a tiempo completo, en la ciudad de Salvador, Bahía. La Escala de autoestima de Rosenberg y el Body Shape Questionnaire evaluarón, respectivamente, la autoestima y la insatisfacción con la imagen corporal. También se evaluaron los datos demográficos y socioeconómicos, la maduración sexual, el estado antropométrico, el nivel de actividad física y la inactividad física. La asociación entre la insatisfacción con la imagen corporal y la autoestima se analizó mediante regresión multinominal para variables politómicas utilizando el programa estadístico SPSS Versión 23.0. Resultados: Entre los adolescentes, el 24,8% tenían una moderada autoestima y el 27,3% tenían las puntuaciones más bajas de este constructo. La insatisfacción con la imagen corporal se identificó en el 25,2% de los adolescentes. El análisis de asociación reveló que las chicas y los chicos con la insatisfacción con la imagen corporal tenían, respectivamente, 5,61 (IC 95% 3,08 - 10,24) veces y 3,83 (IC 95% 2,03 - 7,25) veces en tener más chances de tener baja autoestima. También entre las chicas, la insatisfacción con la imagen corporal se mantuvo positivamente asociada con uma moderada autoestima después del ajuste del modelo (OR: 2,31; IC del 95%: 1,21 a 4,41). Conclusión: Se observó una alta ocurrencia de baja y moderada autoestima y que la insatisfacción con la imagen corporal influyó en la baja autoestima en ambos sexos y en la moderada autoestima entre las chicas.


ABSTRACT. Objective: To analyze an association between dissatisfaction with body image and self - esteem of adolescents enrolled in the public elementary school system in Salvador - Bahia. Methods: Participated in this cross-sectional study 860 adolescents aged 10 to 19, of both sexes from the 7th to the 9th year, enrolled in twelve medium-sized schools of the public elementary school system in full shift, in the city of Salvador, Bahia. Self-esteem was assessed through the Rosenberg Self-esteem Scale and body image dissatisfaction by the Body Shape Questionnaire. Demographic and socioeconomic data, sexual maturation, anthropometric status, physical activity level and sedentary lifestyle were evaluated. The association between dissatisfaction with body image and self-esteem was analyzed by multinominal regression for polytomic variables using the statistical program SPSS Version 23.0. Results: Among the adolescents, 24.8% had moderate self-esteem and 27.3% had the lowest scores of this construct. Body image dissatisfaction was identified in 25.2% of the adolescents. The association analysis revealed that adolescents with body image dissatisfaction have 5.61 (95%ci 3.08-10.24) times greater chance of manifesting low self-esteem for females and 3.83 (95%ci 2.03-7.25) times for males. Still among girls, dissatisfaction with body image remained positively associated with moderate self-esteem after adjusting the model (OR: 2.31; 95%ci 1.21-4.41). Conclusion: It was concluded that the prevalence of low and moderate self-esteem was high, and dissatisfaction with body image influenced low self-esteem in both sexes and a moderate among girls.

This review aimed to systematically assess the effects of computer-based interventions for the promotion of healthy eating on anthropometric indicators and food consumption of adolescents at school. Relevant scientific articles were searched in MEDLINE/PubMed, Embase, Scopus, Lilacs, PsycINFO and Cochrane Library/CENTRAL following the recommendations by PRISMA guide in August 2021. The quality and risk of bias in the studies were assessed by two reviewers independently. The search strategy retained 13 studies for qualitative analysis. In total 9.603 adolescents of both sexes, were recruited in a school environment. According to studies with information for the meta-analysis, a reduction in the average consumption of fats in favor of computer-based intervention was identified (SMD: -0.14; 95% CI -0.24;-0.05). Considering the changes in the consumption of fruits and vegetables (SMD: 0.09; 95% CI -0.08; 0.26) and in the BMI (WMD: -0.02; 95% CI -0.18; 0.14), the analyses did not identify the effect of computer-based interventions on these parameters after follow-up. The results of this review demonstrate that there is evidence of the positive effect of using digital technology (website and CD-ROM) in educational programs with a focus on promoting a healthy lifestyle, with evidence on reducing fat consumption.

Resumo O objetivo do estudo foi analisar a variação temporal da ocorrência do excesso de peso e da obesidade abdominal em adolescentes de escolas públicas de uma capital do nordeste brasileiro. Trata-se de um estudo incluindo informações de três levantamentos transversais realizados nos anos de 2001, 2009 e 2017, com 2.496 escolares de 10 a 18 anos de ambos os sexos, residentes na cidade de Salvador, Bahia. O excesso de peso foi avaliado por meio do Índice de Massa Corpórea para idade (IMC/I) e obesidade abdominal foi avaliada pela circunferência da cintura e razão cintura-estatura. A regressão Pooled OLS (Ordinary Least Squares), de efeito constante foi utilizada para avaliar a variação dos indicadores antropométricos ao longo do período. Os resultados deste estudo indicam que a prevalência de excesso de peso cresceu 103% e os indicadores da obesidade abdominal cresceram 153% e 142%, respectivamente no período analisado. Aumento na média do Índice de Massa Corpórea e da Razão cintura estatura foi maior entre as adolescentes e da média do IMC e da circunferência da cintura entre aqueles de 14 a 18 anos de idade. Conclui-se que houve expressivo crescimento nas prevalências do excesso de peso e da obesidade abdominal no período de 2001 a 2017 entre os adolescentes da cidade de Salvador.

BACKGROUND: Obesity is a chronic complex disease with great prevalence for children all over the world. Characterized for low-grade inflammation associated with several comorbidities such as resistance and type 2 diabetes mellitus (T2DM). OBJECTIVES: To investigate whether genetic variants in IL10, IL1RL1, IL1B, IRF4, TNF, IL6, and IL33 genes are associated with being overweight in children. METHODS: We performed the genotyping of 1004 children using Illumina 2.5 Human Omni bead chip, and association analysis on the genetic variants and the overweight through logistic regression adjusted for sex, age and components principal. RESULTS: Of the seven genes analyzed, 16 SNVs significantly associated. Eleven variants in IL1RL1, two in IL1B and one in IRF4 genes increased overweight risk and two SNVs in IL1RL1 were associated with protection against overweight. The rs2287047-A was negatively associated (OR: 0.66, CI95%: 0.19-0.45) and had a reduced IL1RL1 expression in whole blood (p 0.033) in silico eQTL. The rs12203592-T, in IRF4, was positively associated with being overweight, and led to an increased gene expression in whole blood (p < 0.001) and adipose tissue (p < 0.001). CONCLUSION: These results suggest that genetic variants in inflammatory genes may play an important role in the development of overweight in children.

BACKGROUND: Preterm birth (<37 weeks), low birth weight (LBW,<2500g), and small for gestational age (SGA,<10th centile of birth weight for gestational age and sex) are markers of newborn vulnerability with a high risk of mortality. We estimated the prevalence of phenotypes combining these three markers and quantified the mortality risk associated with them. METHODS: Population-based cohort study using routine register-based linked data on all births and deaths in Brazil from January 1, 2011, to December 31, 2018. We estimated the prevalence of preterm, LBW, and SGA individually and for phenotypes combining these characteristics. The mortality risk associated with each phenotype: early neonatal, late neonatal, neonatal, post-neonatal, infant, 1-4 years, and under five years was quantified using mortality rates and hazard ratios (HRs) with 95% confidence interval (CI) were estimated using Cox proportional hazard models. FINDINGS: 17,646,115 live births were included. Prevalence of preterm birth, LBW and SGA were 9.4%, 9.6% and 9.2%, respectively. Neonatal mortality risk was 16-fold (HR=15.9; 95% CI:15.7-16.1) higher for preterm compared to term, 3 times higher (HR=3.4; (95% CI:3.3-3.4) for SGA compared to adequate for gestational age (AGA), and >25 times higher for LBW (HR=25.8; (95% CI:25.5-26.1) compared to normal birth weight (NBW). 18% of all live births were included in one of the small vulnerable newborn phenotypes. Of those 8.2% were term-SGA (4.7%NBW, 3.5%LBW), 0.6% were term-AGA-LBW, 8.3% preterm-AGA (3.8%NBW, 4.5%LBW) and 1.0% preterm-SGA-LBW. Compared to term-AGA-NBW, the highest mortality risk was for preterm-LBW phenotypes (HR=36.2(95%CI 35.6-36.8) preterm-AGA-LBW, HR=62.0(95%CI 60.8-63.2) preterm-SGA-LBW). The increased mortality risk associated with vulnerable newborn phenotypes was highest in the first month of life, with attenuated but continued high risk in the post-neonatal period and 1-4 years of age. INTERPRETATION: Our findings support the value of using more detailed phenotypes to identify those at highest risk. More granular data can inform care at the individual level, advance research, especially for prevention, and accelerate progress towards global targets such as the Sustainable Development Goals. Importance: Despite progress toward reducing global incidence, leprosy control remains a challenge in low- and middle-income countries. Objective: To estimate new case detection rates of leprosy among household contacts of patients with previously diagnosed leprosy and to investigate its associated risk factors. Design, Setting, and Participants: This population-based cohort study included families registered in the 100 Million Brazilian Cohort linked with nationwide registries of leprosy; data were collected from January 1, 2007, through December 31, 2014. Household contacts of patients with a previous diagnosis of leprosy from each household unit were followed up from the time of detection of the primary case to the time of detection of a subsequent case or until December 31, 2014. Data analysis was performed from May to December 2018. Exposures: Clinical characteristics of the primary case and sociodemographic factors of the household contact. Main Outcomes and Measures: Incidence of leprosy, estimated as the new case detection rate of leprosy per 100 000 household contacts at risk (person-years at risk). The association between occurrence of a subsequent leprosy case and the exposure risk factors was assessed using multilevel mixed-effects logistic regressions allowing for state- and household-specific random effects. Results: Among 42 725 household contacts (22 449 [52.5%] female; mean [SD] age, 22.4 [18.5] years) of 17 876 patients detected with leprosy, the new case detection rate of leprosy was 636.3 (95% CI, 594.4-681.1) per 100 000 person-years at risk overall and 521.9 (95% CI, 466.3-584.1) per 100 000 person-years at risk among children younger than 15 years. Household contacts of patients with multibacillary leprosy had higher odds of developing leprosy (adjusted odds ratio [OR], 1.48; 95% CI, 1.17-1.88), and the odds increased among contacts aged 50 years or older (adjusted OR, 3.11; 95% CI, 2.03-4.76). Leprosy detection was negatively associated with illiterate or preschool educational level (adjusted OR, 0.59; 95% CI, 0.38-0.92). For children, the odds were increased among boys (adjusted OR, 1.70; 95% CI, 1.20-2.42). Conclusions and Relevance: The findings in this Brazilian population-based cohort study suggest that the household contacts of patients with leprosy may have increased risk of leprosy, especially in households with existing multibacillary cases and older contacts. Public health interventions, such as contact screening, that specifically target this population appear to be needed.

INTRODUCTION: Overweight and obesity in children is an important global problem. Its prevalence is increasing in developed and developing countries. OBJECTIVE: The aim of this study was to evaluate the association between socioeconomic conditions, maternal body mass index (BMI), food security, and intake of obesogenic foods on excess body weight in children. METHODS: A cross-sectional study was conducted, using the data of 3,676 children from the latest National Demographic and Health Survey in Brazil. The children's BMI was the study outcome. Socioeconomic condition, maternal BMI, food security, and intake of obesogenic foods were use as predictors. structural equation models were used for analysis. RESULTS: Socioeconomic conditions directly influenced the children's BMI (ß = 0.102; p = 0.02), mediated by intake of obesogenic foods (ß = 0.018; p = 0.04). A direct association was observed between maternal and child BMIs (ß = 0.169; p < 0.001) and intake of obesogenic foods and child BMI (ß = 0.114; p < 0.001). CONCLUSIONS: Favorable socioeconomic conditions, increased maternal BMI, and intake of obesogenic foods contributed to increased child BMI.

OBJECTIVE: Identify the factors associated with asthma symptoms in adolescents. METHODS: Cross-sectional study with the participation of 1,176 students between the ages of 11 and 17 years old, in Salvador, Bahia, Brazil. The asthma symptoms were identified by using the standardized questionnaire from The International Study of Asthma and Allergies in Childhood Program (ISAAC), phase III. The subjects' food intake data were collected by a semiquantitative food frequency questionnaire (FFQ) comprised of 97 food items. All of the information was collected during a single interview. Anthropometric, socioeconomic, and demographic data were collected from the study population. The robust Poisson model was adopted to obtain the raw and adjusted PR (prevalence ratio). RESULTS: From the total number of adolescents, 57.6% are female and 83.71% of them were between 13 and 17 years old. The prevalence of asthma symptoms was of 7.6%, being higher among males (9.62%). The prevalence of wheezing in the last 12 months was 8.6%. Among the adolescents with rhinitis and eczema, the prevalence of asthma symptoms was of 15.2 and 14.3%, respectively. After an adjusted analysis, the following factors were identified: females PR = 0.64 (95%CI 0.42 - 0.96), rhinitis PR = 3.23 (95%CI 2.17 - 4.83) and the 2nd tertile of the healthy pattern (moderate consumption) PR = 0.61 (95%CI 0.37 - 0.99). CONCLUSION: Rhinitis and eczema were presented as risk factors for asthma symptoms. The healthy food intake pattern that had been stratified in tertiles, presented the 2nd tertile as a protector for asthma symptoms.

OBJECTIVE: To estimate factors associated with double burden of nutritional outcomes in the mother-child dyad at the household level (child stunting and/or maternal overweight). DESIGN: Cross-sectional study using the Brazilian Demographic and Health Survey. Nutritional outcomes were: mother with normal weight and child with normal height; overweight mother and child with normal height; mother with normal weight and short-stature child; and overweight mother and child with short stature (double burden). The child was classified as short when height-for-age Z-score was <-2 and the mother as overweight when BMI was ≥25·00 kg/m2. Socio-economic status, environment, social vulnerability, maternal characteristics and the child's food intake were the exposure factors. The hierarchical approach for multinomial logistic regression modelling was used to assess the associations. SETTING: National Demographic and Health Survey of Children and Women conducted in Brazil, 2006-2007. SUBJECTS: Mother-child dyads (n 3676). RESULTS: After adjustments, lower maternal educational level (OR=3·53; 95 % CI 1·33, 9·33) and inadequate household (non-masonry house; OR=2·54; 95 % CI 1·39, 4·66) were associated with the double burden of malnutrition. Mother's short stature (OR=3·41; 95 % CI 1·76, 6·61), child's vegetable intake on less than or equal to 4 d/week (OR=2·21; 95 % CI 1·03, 4·75) and inadequate household (non-masonry house; OR=2·29; 95 % CI 1·36, 3·87) were associated with child's short stature. The lack of breast-feeding (OR=2·00; 95 % CI 1·07, 3·72) was associated with maternal overweight. CONCLUSIONS: The present findings contribute to establishing strategies promoting health and healthy diets, by considering the growth deficit and overweight/obesity concomitantly.

Objective: This study assessed the validity of a Food Frequency Questionnaire used to assess food intake in adolescents in Salvador, Bahia, Brazil. Methods: Seventy adolescents enrolled in public schools aged 11 to 17 years participated in this study. The dietary intake of the adolescents was assessed using the Food Frequency Questionnaire and the mean value of three-day food record, used as the reference method. The mean (and standard deviation) energy and nutrient intakes estimated from the Food Frequency Questionnaire and food records were calculated. The paired Student's t test was used to determine differences between the data. Results: Pearson's correlation coefficient and Kappa statistics were used to measure the strength of the association between the two instruments. There was variation in the crude unadjusted (r=0.61-0.73) and deattenuated (r=0.33-0.99) Pearson correlation coefficients. After adjusting for energy, the crude and deattenuated coefficients ranged from r=0.53-0.81 and r=0.27-0.99, respectively. The intake of energy and most nutrients were found to be adequate, except for fiber (r=0.27) and calcium (r=0.33), which showed a weak correlation. Kappa statistics showed good correlation between all values varying from r=0.47 to 0.73. Conclusion: It is considered that an average r value greater than 0.40 indicates a good correlation, the results obtained indicate the good ability of the Food Frequency Questionnaire to assess individuals according to their usual intake of most nutrients.


Objetivo: Verificar a validade do Questionário de Frequência Alimentar utilizado para avaliar o consumo de alimentos dos adolescentes em Salvador, Bahia. Métodos : Participaram do estudo 70 adolescentes, com idade entre 11 e 17 anos, matriculados nas escolas da rede pública. Seu consumo alimentar foi avaliado pelo Questionário de Frequência Alimentar e pela média de três registros alimentares como método de referência. Foram estimadas as médias (desvio-padrão) da energia e dos nutrientes extraídos do Questionário de Frequência Alimentar e dos registros alimentares. Para verificar as diferenças foi aplicado o teste t Student pareado. Para medir a concordância entre os dois instrumentos foram calculados os coeficientes de correlação de Pearson e estatística de Kappa. Resultados: A análise dos coeficientes de correlação de Pearson não ajustados brutos, foi possível observar uma variação entre (r=0,61-0,73), já nos deatenuados entre (r=0,33-0,99). Após ajustados por energia os valores brutos variaram entre (r=0,53-0,81) e os deatenuados (r=0,27-0,99), observou-se valores adequados para energia e a maioria dos nutrientes, com exceção da fibra (r=0,27) e do cálcio (r=0,33), que apresentaram fraca correlação. No método de Kappa, todos os valores obtidos na análise apresentaram boa correlação, variando entre (r=0,470,73). Conclusão: Considera-se que a média dos coeficientes para boa correlação deve estar acima de 0,40, esses resultados indicam a capacidade do Questionário de Frequência Alimentar em classificar os indivíduos segundo seu consumo habitual para a maioria dos nutrientes.

RESUMO: Objetivo: Identificar os fatores associados a sintomas de asma em adolescentes. Métodos: Estudo transversal, com a participação de 1.176 alunos com idade entre 11 e 17 anos, em Salvador, Bahia, Brasil. Os sintomas de asma foram identificados utilizando-se o questionário padronizado do The International Study of Asthma and Allergies in Childhood Program (ISAAC), fase III. A avaliação do consumo alimentar dos adolescentes foi obtida por meio de questionário da frequência alimentar (QFA) semiquantitativo, com 97 itens alimentares, sendo as informações coletadas em uma única entrevista com os estudantes. Foram coletados dados antropométricos, socioeconômicos e demográficos da população do estudo. O modelo de Poisson robusto foi adotado para obtenção da razão de prevalência (RP) bruta e ajustada. Resultados: Do total de adolescentes, 57,6% são do sexo feminino e 83,7% tinham de 13 a 17 anos de idade. A prevalência de sintomas de asma encontrada foi de 7,6%, sendo mais elevada no sexo masculino (9,62%). A prevalência de chiado nos últimos 12 meses foi de 8,6%. Entre os adolescentes que tinham rinite e eczema a prevalência de sintomas de asma foi de 15,2 e 14,3% respectivamente. Após análise ajustada, os seguintes fatores se associaram à asma: sexo feminino RP = 0,64 (IC95% 0,42 - 0,96), rinite RP = 3,23 (IC95% 2,17 - 4,83) e o 2o tercil do padrão saudável (consumo moderado) RP = 0,61 (IC95% 0,37 - 0,99). Conclusão: Rinite e eczema apresentaram-se como fatores de risco para sintomas de asma. O padrão alimentar saudável estratificado em tercil apresentou o segundo tercil como protetor para sintomas de asma.

OBJECTIVE: This study identifies the risk factors for extreme weight-control behaviors among adolescents in public school in Salvador, northeastern Brazil. METHODS: A case-control study nested to a cross-sectional study, including 252 adolescents of both sexes, age between 11 to 17 years, with 84 cases and 168 age-matched controls was conducted. The variable outcome is represented by extreme weight-control behaviors, integrated by following the variables: self-induced vomiting and the use of laxatives, diuretics, or diet pills. Covariables included body image dissatisfaction, dieting, prolonged fasting, and self-perception of body weight. The study also investigated the demographic and anthropometric variables and economic conditions of the students' families. Conditional logistic regression was used to identify risk factors for the adoption of extreme weight-control behaviors among adolescents. RESULTS: Among the adolescents investigated, the conditional logistic regression explained 22% the occurrence of extreme weight-control behaviors and showed that these behaviors were positively associated to overweight (odds ratio [OR] = 3.61; 95% confidence interval [CI], 1.42-9.17), body image dissatisfaction (OR = 3.87; 95% CI, 1.75-8.54), and the adoption of a restrictive diet (OR = 2.83; 95% CI, 1.16-6.91). CONCLUSIONS: The results of this study suggest that among adolescents, overweight, body image dissatisfaction, and restrictive diet are important risk factors to adoption of extreme weight-control behaviors.

AbstractThe aim of this study is to evaluate the influence of dietary patterns on the occurrence of dental caries. Using a population-based, cross-sectional study design, we assessed a sample of 1.439 children aged 6-13 years who were enrolled in public schools in the municipality of São Francisco do Conde, Bahia state, Brazil. Oral examinations for dental caries were performed by two dental surgeons (Kappa = 0.72) in the school environment under natural light with the aid of a wooden spatula and gauze. Dietary intake was assessed using the qualitative-quantitative food frequency questionnaire. A questionnaire on the demographic and socioeconomic conditions was applied to the families. The Poisson multivariate analysis was performed for statistical purposes. Considering both dentitions, the prevalence of caries was 82.14% and 37.3% in the permanent and deciduous dentitions, respectively. The identified dietary patterns were classified into “standard obesogenic" and "prudent." The results showed statistically significant association between the obesogenic dietary pattern and dental caries; the proportion of students with dental caries was 1.4 times higher among those who most frequently consumed foods that constitute a standard obesogenic diet (PR = 1.40, 95% CI: 1.04-1.96) compared to those who less frequently consumed these types of foods. A positive association between the obesogenic dietary pattern and dental caries was found in the students investigated. Therefore, the promotion of healthy eating habits and nutritional habits should be encouraged to prevent dental caries in children and adolescents.


ResumoO objetivo deste estudo é avaliar a influência de padrões alimentares sobre a ocorrência de cárie dentária. Realizou-se um estudo transversal de base populacional, com uma amostra de 1.439 crianças de 6 a 13 anos, matriculadas em escolas públicas da cidade de São Francisco do Conde, no Estado da Bahia. Os exames de cárie dentária foram realizados por dois cirurgiões-dentistas (Kappa=0,72) no ambiente escolar, sob luz natural, com o auxílio de uma espátula de madeira e gaze. A ingestão alimentar foi avaliada por meio do questionário qualitativo-quantitativo de frequência alimentar. Um questionário sobre as condições demográficas e socioeconômicas foi aplicado nas famílias. A análise multivariada de Poisson foi realizada para fins estatísticos. Considerando ambas as dentições, as prevalências de cárie foram 82,14 e 37,3% nas dentições permanentes e decíduas, respectivamente. Os padrões alimentares identificados foram classificados em "padrão obesogênico", e os resultados mostraram uma associação estatisticamente significativa entre o padrão alimentar obesogênico e a cárie dentária "prudente". A proporção de alunos com cárie foi 1,4 vez maior entre aqueles que consomem frequentemente alimentos constituídos de uma dieta obesogênica padrão (RP=1,40 e IC95%=1,04-1,96) em comparação com aqueles que consomem, com menor frequência, esses tipos de alimentos.Uma associação positiva entre o padrão alimentar obesogênico e a cárie dentária foi encontrada nos estudantes Portanto, a promoção de hábitos alimentares saudáveis e de hábitos nutricionais devem ser incentivados como forma de prevenção da cárie dentária em crianças e em adolescentes.

OBJETIVO: Este estudo objetivou identificar a soroprevalência da doença celíaca em adolescentes de escolas públicas da cidade de Salvador, Bahia. MÉTODO: Trata-se de um estudo transversal com amostra probabilística de 1.213 adolescentes de 11 a 17 anos, de ambos os sexos. O índice de massa corporal foi utilizado para o diagnóstico do estado nutricional, adotando-se os percentis segundo idade e sexo, propostos pela World Health Organization. O anticorpo anti-transglutaminase humana da classe imunoglobulina A (anti-tTG-IgA) foi adotado como teste sorológico para triagem da doença celíaca e foi determinado pela técnica do ensaio imunoabsorvente ligado à enzima (ELISA). Foi realizada análise descritiva, utilizando-se a proporção e a média (desvio padrão). RESULTADOS: O sexo feminino predominou entre os adolescentes, e a maioria encontrava-se com adequado estado nutricional. O anticorpo anti-tTG-IgA foi positivo em 6/1.213 (0,49%) adolescentes. CONCLUSÃO: A soroprevalência de doença celíaca entre os adolescentes estudados foi 0,49%. Novas investigações são necessárias para confirmar a prevalência de doença celíaca nessa faixa etária. .

The aim of this study was to assess the relationship between eating patterns and body mass index (BMI) in children and adolescents. This is a cross-sectional study of 1,247 male and female students, aged between 6 and 12, from public elementary schools in São Francisco do Conde, Bahia State, Brasil. BMI was used to analyze the children's nutritional status. Food consumption frequencies, in addition to demographic and socioeconomic information, were collected for each participant. Dietary patterns were identified through a factor analysis. The prevalence of overweight and obesity was 17.3% (10.2% overweight and 7.1% obese). Two eating patterns, "obesogenic" and "prudent", were identified. The former is characterized by sweets and sugars, typical Brazilian dishes, pastries, fast food, oils, milk, cereals, cakes, and sauces, and was positively associated with increased BMI (ßi = 0.244; p = 0.018). An "obesogenic" dietary pattern was associated with increased BMI.
